Dependent Care Flexible Spending Plan The Dependent Care Flexible Spending plan allows Associates to have additional deductions taken from their paychecks pre-tax to be used for dependent care costs. The elected amount is available to the Associate only after the money has been contributed to their account; however, all unused funds are forfeited at the end of the plan year or upon the ending of the employment relationship. Associates may enroll in the plan when initial eligibility requirements are met and make changes annually during open enrollment with changes effective on January 1st of the following year. COBRA Under the Consolidated Omnibus Budget Reconciliation Act of 1986 (“COBRA”), Associates may be entitled to continued healthcare benefits upon their employment ending with Dwellworks. COBRA enables Associates, spouses, and their dependents to continue receiving healthcare coverage in certain circumstances when coverage would otherwise cease. In general, COBRA provides Associates the opportunity to receive the same options for healthcare benefits that the Associates currently have at the group rate should include their employment status with Dwellworks change or if there is a change in their relationship with their family. COBRA applies only to healthcare coverage. Associates may become eligible for continued healthcare benefits at current group rates if they or their beneficiaries fall into one of the following categories. The duration of coverage depends on the condition of eligibility. Specific rates and guidelines can be obtained through Human Resources. Associates (spouses and dependent children) who have been terminated 18 months of coverage Associates (spouses and dependent children) if the Associate’s hours have been reduced resulting in the loss of coverage 18 months of coverage A widowed spouse and dependent children up to age 26 36 months of coverage A divorced or legally separated spouse and their dependent children 36 months of coverage A spouse who is Medicare ineligible 36 months of coverage Dependent children who no longer meet the eligibility status 36 months of coverage Medical Benefits Abroad Associates traveling on business outside their country of origin receive medical insurance coverage through the Dwellworks Medical Benefits Abroad (“MBA”) plan. The MBA plan covers personal travel for an Associate and his/her dependents for up to seven (7) days when personal travel is taken in combination with business travel. The MBA plan covers the following: + Medical appointments and medical care due to illness; + Ambulance service and emergency care; + Outpatient care, inpatient care, and hospital admissions; + Prescription drugs and replacement medicine for lost prescriptions that are medically necessary during the time of your travels; and + Medical evacuation in the event that an Associate requires immediate medical attention and adequate facilities are not locally available. The MBA does not cover wellness appointments or maintenance prescription refills. Life Insurance Dwellworks offers a complimentary life insurance policy to qualifying Associates in the amount of $50,000 for group term life and $50,000 for accidental death and dismemberment. To qualify, an Associate must work at least thirty (30) hours per week. Qualifying Associates are eligible to receive this benefit beginning on the first (1st) day of the month following the date of hire. Associates are automatically enrolled in the plan when eligibility requirements are met. Associates may enroll in optional life insurance plans when initial eligibility requirements are met at an additional cost.
