Perks Associates can flex their hours by… + Start work between 7:30am – 9:30am + Taking lunch for 30 or 60 minutes + End work between 4:30pm – 6:30pm After working for six months at Dwellworks, associates can work remotely for 12 days or can be taken in ½ increments

Our Enrollment Process
About Making Changes + Effective the first of month following date of hire + Can make changes to elections when… - Eligible for all benefits the 1 st of the month after start date - In the event of a qualified life event... Within 30 days of the event - During our annual open enrollment (effective 1/1 of the following year) What Dwellworks Does + Negotiates affordable rates with the benefit providers + Looks at plan costs and evaluate each year + Communicates any changes during open enrollment process

Examples of a co-pay include office visits, Rx, urgent care, emergency room, and other services outlined in the summary of benefits and summary plan descriptions. Although a co-pay is excluded from the deductible, it is included in the out-of-pocket maximum. For a deductible, apply only to covered medical services listed with a percentage in the summary of benefits and summary plan descriptions. On the PPO plans, the single deductible applies to each individual enrolled in the EE+children or family plans with no more than three family members maxing out the deductible. The coinsurance eligible services are noted in the summary of benefits with notation of a percentage, and the costs will be applied to deductible. Once the deductible is satisfied, the associate will share in the service’s cost.
The out-of-pocket maximum is the amount an associate will pay annually for services.

CO-PAY
COINSURANCE
A co-pay is a flat dollar amount paid for services as part of the Preferred Provider Organization (PPO) plans.
A coinsurance is the percentage of negotiated costs that associates pay for eligible services.
A deductible is the annual amount that must be satisfied prior to coinsurance percentages being applied. DEDUCTIBLE

+ Those enrolled in Option 1 (HDP/HSA) have the option of opening a Health Savings Account + OptumUHC is our HSA provider + The HSA account allows you to put money aside pre-tax + 2019 Maximums $3,500 Single/ $7,000 Family + Additional $1,000 for Single and Family if 55 and older + The amount you set aside can be changed each pay + You can only use money that you have in the account + The money can accumulate in the account from year to year + HSA money can only be used for eligible expenses

+ Maximum contribution amount is $2,700 annually in 2019 + Contributions through payroll in equal increments each pay + Can use the money in advance of it being in your account + You can apply for a debit card, submit claims online, or submit paper claims + Must incur claims during the claim period to receive reimbursement + Account is terminated upon termination date from Dwellworks

- If you elect EE 100,000 you can add: 50,000 Spouse and 10,000 Child
